The Homeless Service Team provides intensive case management services to single adults who meet the definition of long-term homeless.
Services offered:
· Outreach, engagement, relationship-building · Locate adequate housing · Ensure that basic human needs are met · Facilitate application and appeals for benefits · Help to manage and avoid crisis situations · Counsel, train, and provide information for employment · Provide and connect to mental and physical health care and support services |
